It is with great thanks to David and Liz Page, writers and publishers of the Mountain Home Cemetery, Daisy, Oklahoma for giving permission to post the surname index to this book. And to all the others that helped record the names and dates in this cemetery. They regret that the cemetery wasn't recorded years ago. Many peoples names and dates have been lost to time and neglect. The book is currently being updated every 2-4 years. [2003]

All proceeds from the sale of books going to the Daisy Mountain Home Cemetery Association. All proceeds are used for the upkeep and improvements of the cemetery.
