Step By Step Instructions For Completing Application Papers

Application may printed above, and will fit on a 8½ x 11 in paper,  or from Craig County Genealogical Society . Application papers my also be completed in black ink. (please, print.). Be sure to proof read all papers, and signed the the top page.

Page 1: Cover Page - Before Statehood Application, After Statehood Application

  1. Leave Membership Number,  Received, Verified, Examined blank. Only fill in the Bold Areas.
  2. Fill in full name and address of person making this application. (women include maiden name and put in parentheses ( SMITH )  ) .
  3. Write the name of the ancestor you are honoring. This can be both husband and wife if married at the time they lived in the area. (Such as James Hoolie Doe and Mary Jamina Noo Doe).
  4. Residence such as Needmore, Delaware District or Vinita, Coo-wee-scoo-wee District.
  5. Occupation is what ever your ancestor was doing to earn a living when they resided in the area.
  6. Signature of Application and date: Application without a hand written signature and date will not be excepted.
  7. Please read sign and date if we may use your name a submitter of this paper.
  8. Leave lower section blank.
     

Page 2-5: Lineage

  1. Throughout the application, use the full or legal name. Do not use initials alone unless this was the person only given mane. A nickname may be written in “Quotes”. A woman is referred to by her maiden name, (For generation 2 and beyond if a widow or divorced at time of this marriage as Mrs. first, middle, maiden name, surname, ie.. Mrs. Ann Jo Smith Doe.)
  2. Names, dates and places must be complete with proof that will clearly tie each generation to the preceding one. If a date or place cannot be secured for the first three generations, please furnish and explanatory statement.
  3. Cite places as town, County or parish and state. (Use common postal abbreviations for states.) For Cherokee Nation Indian Territory give the town and district, IT.
  4. Be sure each name, date and place agrees with or is confirmed by the proof submitted. If it does not, send a letter of explanation.

Generation #1: The Applicant
       
Provide your full name. If married give husbands full name and date of marriage. Note: If an adoption is involved put adopted in parentheses after the name of the adopted child.

Generation # 2: The Applicant’s Parents
        Complete the phrase, “I am the child of……..” first with the Father’s full name, birth date and place, and death date and place, If applicable. Complete the marriage date of parents (If never married put N/A). Following To give mother’s full name, birth date and place, and death date and place.

Generation #3: The Applicant’s Grand parents
       
Give name of parent who is the descendant of the ancestor with all appropriate names, dates and places. If a full name, date or place cannot be secured the applicant should furnish and explanation.

Generation $4: the Applications Great-Grand parents
        Give name of parent who is the descendant of the ancestor with all appropriate names, dates and places. If a full name, date or place cannot be secured the applicant should furnish and explanation.

Generation #5 - X  Through to the Ancestor:
        As above, list all names, dates and places to clearly identify each generation of the direct line. If need more charts reprint this form .

PROOFS: (References, sources, documents or evidence used to prove the applicant’s lineage on all pages.)

  1. Proof is requested for all dates, places and relationships given for lineage
  2. For proof use the best available source
  3. Family tradition is not proof. Pedigree charts, family group sheets, undocumented genealogies are not proof.
  4. Send enough documentation to clearly identify each name, date, place and family relationship for all generations.
  5. Lift proof for each generation. Send on copy of the proof listed even if it is cited more than once.
  6. Send photocopies of all proofs. DO NOT SEND ANY ORIGINALS.

Last Page: Children

Children of the ancestor:
        List all know children of the earliest ancestor and birth dates and to whom married if know.

Proof of Residents:
       


Include a $25.00 check for application fee and a legal size SASA for conformation or if society need to request more information.

 If needed more the four generation. Print out another Generation form and write in the next generation (ex. 5, 6, 7 etc..) . If their is more then 15 children from your earliest ancestor print out another form and finish listing the rest of the Children.
