Instructions for Submitting Your Family Groupsheet
The form below is made for a family of 15 children. To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname. We hope that you will also submit a separate chart on the mother's family. If there is more than one marriage, submit a second family groupsheet and make a notation in the documentation field that states this is an additional marriage. Use your Tab key to jump between fields, the Enter key will submit the record in newer browsers. If this happens, use your browser's "Back" button to return to your record.

Put all SURNAMES in capital letters! Spell out all dates, cities, and states.
DO NOT include LIVING PERSONS on these charts. You can use Living SURNAME.
Add any additional information in the area left for notes, below the children.
We strongly encourage everyone to quote their sources in the notes area!!!
Family must show a connection to Oklahoma. Any submissions that do not, will be ignored.
