PHYLLIS ERVIN

Phyllis J. Ervin, 49, Tulsa, died Tuesday at Hillcrest Medical Center.

Services will be 1 pm Monday at Friendship Baptist Church, 1709 N. Madison, Tulsa.

Burial will be at North Heights Cemetery, Sapulpa, under the direction of jack’s Memory Chapel, Tulsa.

She was born Feb 23, 1939, in Sapulpa.

Survivors include mother Carrie Ervin; daughter Johnsetta of Sapulpa; son James of Tulsa; sisters Mary Zachary of Sapulpa, Fannie Mackey and Hazel Ervin, both of Tulsa, Willie B. Allen, Tempie Ervin, Minnie Ervin, of Aurora, Ill.; brother James Ervin Jr., of Aurora; and two grandsons.

The family will headquarter at the Hazel Ervin Residence, 1518 W. 59th, Tulsa.
